The Oak Trace Health Center was recently recognized as a five-star health services and skilled nursing provider. The prestigious recognition from the Centers for Medicare & Medicaid Services and U.S. News & World Report was given to only the top 21 percent of senior health care providers in Illinois. Ratings are based on surveys for quality measures, staffing and on-site inspections.
The health inspection ratings contain information from the last three years of on-site inspections, which are conducted unannounced every year at health care and skilled nursing centers throughout the country.
“We work to deliver the highest level of care and a holistic approach to the health needs of our residents,” said Michelle Hart-Carlson, administrator of Oak Trace Health Services. “Receiving a five-star rating is a reflection of our team’s expertise and dedication, and I’m proud to see their efforts recognized on a state and national level.”
Senior health care and skilled nursing providers have more than 200 regulations to follow every day, leading to a potential for experiencing minor human errors.
“We’ve worked diligently to be honored as one of the best skilled nursing providers,” said Hart-Carlson. “And we’re going to continue to work hard to maintain our five-star rating, and make sure our residents and their families are happy.”
